If you have already scheduled an appointment with us, have provided us with your email address and have accepted your invitation to Breeze, you have the option of filling out your patient forms with us electronically via Breeze.
Regarding you Breeze invitation, be sure to check your messages in the e-mail account that you have on file with us, including your spam messages, for a message from messages@carecloud.com. If you have not received an invitation, please contact the office at (813) 880-SKIN (7546) so that we can send you an invitation.
